HIV Case Management Services
What is Case Management?
Better Home Health Care HIV Case Management Department provides linking, coordinating and arranging of clients
to community resources to include but not limited to:
- Access client's needs for services and provide counseling accordingly, including planning with regard to medical, housing and other psychosocial issues.
- Develop and maintain treatment plans with clients.
- Maintain documentation including case conferences, progress notes and correspondence.
- Establish linkages with other community based organizations and work toward improving service coordination, access, and delivery of services.
- Conduct home visits to determine process towards independence.
